Specific Treatment of Typhoid Fever

Antibiotics in Treatment of Typhoid Fever

Ciprofloxacin 500 mg (30mg per Kg body weight 12 hourly for 3 days in children) oral morning and evening(BD) for 14 days.

OR

Ceftriaxone 1 gram IV in morning and evening (BD) for 7 days.

OR

Levofloxacin 500 mg (30mg per Kg body weight 12 hourly for 3 days in children) oral morning and evening(BD) for 14 days. or Levofloxacin 750 mg once daily for 14 days.

Multi-Drug Resistant Typhoid Fever antibiotics

Ceftriaxone 2 grams IV in morning and evening (BD) for 10 to 14 days

OR

Azithromycin 500 mg IV Once a day (OD) for 14 days

Treatment of Carrier Typhoid fever

Ciprofloxacin 500mg or 750 mg oral in morning and evening (BD) for 4 weeks.

OR

Norfloxacin 400mg oral in morning and evening (BD) for 28 days(4 weeks)

or administer antibiotics according to drugs susceptibility .

Cholecystectomy may also cure.

Symptomatic Treatment of Typhoid Fever

Patient should be admitted in hospital who is unable to take orally with sever vomiting and/or dehydration .

IV infusions preferred is Ringer lactate .

Add KCl (Potassium Chloride) in infusion: Should only be added in case of hypokalemia. Oral correction can not be done rapidly. Direct IV injection should be prevented instead IV with infusion should be used. Adult does 40 mmol (3 ampules of 10ml and 10% KCL, containing 13.4 mmol/ampules) should be added in Ringer Lactate 1 litter over 4 hourly should not exceed 10 mmol/hour. Child Dose over 1 month 0.2 mmol/kg/hr over 3 hours . Each mmol of KCL diluted in 25 ml of Ringer lactate. Check for bradycardia or tachycardia from radial pulse.

Add glucose in infusion if needed.

give all these urgently (STAT) in case of sever dehydration.

Treatment of Fever in Typhoid Fever

Drink plenty of water.

Avoid concentrated fluids like tea , coffee and alcohols as these can increase dehydration.

Avoid taking cold bath as that can increase vasoconstriction in blood vessels of skin . When blood vessels bring blood to the skin heat is emitted from skin . So if blood vessels are constricted heat traps in core body organs which can increase organs damage. Moreover cold causes shivering which can increase production of heat .

Make sure you take plenty of rest especially bed rest.

If you have log hairs and live in warmer area than you may need a hair cut to easily dissipate heat from your head.
